Understanding your needs and goals
The assessment process gathers information about your background, current circumstances, and future goals related to your gender identity.
You will be asked a series of questions to understand your individual needs and how the clinic can best support you.
The goal is to create a clear picture of your situation and tailor the care provided to your specific circumstances.
- Most assessments take place over a video call
- If you haven't yet transitioned socially, the clinician may discuss potential challenges and help you think about ways to address them
- This could include providing information about different aspects of transitioning, such as changing your name, pronouns, or presentation.
Your assessment will help determine the most appropriate next steps in your gender care. Depending on your individual needs and goals, this may involve:
- Referrals to other services within the clinic, like voice therapy, psychological therapy, hair removal, or surgery
- Referrals to local services through your GP, such as support groups or additional therapy options
- The clinic working with you to develop an individualised treatment plan that aligns with your desired outcomes.
At Transcend, we operate a fully virtual model of assessment, ensuring flexibility, convenience, and accessibility for all our clients. Below, you'll find essential information to help you prepare for your online appointment.

Appointment duration: Initial assessments typically take around 90 minutes.
Privacy matters: Please ensure you are in a private location with a stable Wi-Fi connection.
Support person: If you'd like someone to join you during the assessment, that's absolutely fine—just let us know in advance.

Top Tips for a smooth consultation
Safe environment: Make sure you're in a secure and private space.
Minimize interruptions: If you're in an environment where interruptions might occur, please let us know beforehand.
Steady camera: Keep your camera or phone stable to avoid unnecessary movement.
Face visibility: Ensure your face is clearly visible on camera, avoid pointing the camera at ceilings or away from your face.
Prepare ahead: Take some time to think about what you hope to gain from your consultation. Writing down key points can help ensure nothing gets missed.
We understand that communication styles and needs vary. If you find certain types of language or questions challenging, please let us know in advance. We'll also send a pre assessment leaflet, which offers space to share any specific requirements.
Sometimes, it may take several appointments to gather all the necessary information at the start of your journey. Don’t worry, this is entirely normal, and we are fully prepared for this.
As our service operates virtually, physical examinations are neither possible nor typically necessary. With your consent, we can securely access your GP records. This allows us to review key health information, such as blood pressure readings taken by your GP practice nurse or pharmacy.
Your comfort and clarity are our priorities. If you have any questions or need further assistance, don’t hesitate to reach out.
We look forward to supporting you on your journey with Transcend.
Forms of hormone administration
Oral medication: Some hormones can be taken orally and processed safely by the liver. However, this option is generally limited to trans women under the age of 40 with no additional health factors, as these can increase the risk of liver complications.
Monitoring and management
Electronic prescribing: Since our service began, we have used the same electronic prescribing system as general practitioners, allowing medication to begin as soon as the assessment process is complete.
Blood tests: Blood tests may be requested from your general practitioner or ordered directly from a laboratory, depending on your circumstances. Initially, blood tests are required every 12 weeks until stability is achieved.
Ongoing care: We are happy to prescribe hormone therapy for the first 12 months before arranging a transfer of care to your general practitioner.
Endocrinology support: Our service includes access to a consultant endocrinologist who can provide expert advice for more complex hormone management cases.
Your health and safety remain at the forefront of everything we do. If you have further questions, feel free to reach out. We're here to support you every step of the way.
- Waiting Times: As there is a high demand for gender-affirming care in the UK, please be aware that waiting times for assessments and subsequent treatments can be significant
- Accessibility: Transcend strives to offer a flexible range of support options tailored to your individual needs. You will have opportunities to ask questions about your care throughout the process.
