We are now accepting referrals for young adults from 16 to 18 years old. The referral pathway into the service is the same as for adults.
Please ensure the following criteria is met when referring in:
- ECG
- Chest x-ray
- Face to face with GP including physical observations
- Bloods: FBC, U&E, LFT, CRP, CK, B12, Folate, Ferritin,TSH, Serum Calcium, BNP and HbA1c.
Please only refer into the service once all results are available, to avoid rejections.
