Who we are
Our Community Physiotherapy Service provides short term physiotherapy to support patients within their own homes when they are experiencing physical or functional difficulties. For example, we support people who:
- Have a history of falling
- Are at a high risk of falls
- Need a walking aid
We aim to improve your health and wellbeing, educate and enable you to manage your health condition and prevent unnecessary admission to hospital.
We support you to remain at home through assessment, treatment and signposting to other relevant services.
Who is our service for?
Our service is available for anyone over the age of 18 who is registered with a St Helens GP and who are not already receiving physiotherapy support through another service.
Your first appointment will take approximately one hour. You may want to have a friend or family member with you.
At your appointment we will discuss your current abilities, difficulties and needs, and the therapist will assess your muscle strength, balance, walking and ability to move around your home.
Your physiotherapist may also want to look at other activities such as going up and down the stairs or walking outside but this will be discussed with you first.
We will discuss your treatment options, including the pros and cons for each option, and help you to decide your treatment of choice. Treatment usually involves an exercise programme and advice which is tailored to meet your individual goals.
Further visits will be planned to fit in with what is best for you.
You can be referred to the service by your GP, practice nurse or other healthcare professional.
