Patients can be referred to us by your GP, a health professional or you can make a self referral.

Once we receive your referral form, we will contact you to arrange an initial assessment appointment with a specialist podiatrist.

Depending on your assessment, you may be offered a package of treatment. We will write back to your GP to inform them of the outcome of your assessment and any ongoing care.

Please download this self referral form and return your completed application to: Podiatrysouthport@merseycare.nhs.uk

Make sure that the podiatrist has access to your feet, by wearing socks and suitable footwear, and removing any nail polish.

For Biomechanical assessments please ensure you attend wearing shorts or trousers which will roll above the knee.
