Who we are
The service aims to support people to achieve their potential when living with a long term condition and/or cancer through a solution focused approach.
Following an initial appointment patients can decide if they would like another appointment, or may be offered a range of other suitable possibilities such as joining a group, mindful relaxation, or linking up with other services.
The team work closely with the community pain team, stroke services and oncology/palliative care.
The Living Well programme is a five week course run by volunteers with long term conditions with monthly follow up meetings. Patients can self-refer or can be referred by a health or social care professional.

- Monday: 9.00am to 5.00pm
- Tuesday: 9.00am to 5.00pm
- Wednesday: 9.00am to 5.00pm
- Thursday: 9.00am to 5.00pm
- Friday: 9.00am to 5.00pm
- Saturday: CLOSED
- Sunday: CLOSED
Referrals accepted from a GP or patients registered with a Southport and Formby GP. Patients must have one or more health conditions and emotional challenges relating to those conditions. They will be sent information about the service and asked to opt in to a first appointment.
