Our Assessment Team provides specialist assessment, advice and signposting for adults with moderate to severe symptoms of mental illness such as severe depression and anxiety, as well as conditions like schizophrenia and bipolar disorder.
You can contact the team directly for advice or to make a referral on 01925 666 647 (from 8am to 8pm) or 01925 664000 (from 8pm to 8am).
We have developed a single electronic referral form for both the adults’ mental health Assessment Team and the Later Life and Memory Service in Warrington. The form is an electronic PDF with editable text boxes so you can type the information directly into the form fields. Once the form is complete, simply press the ‘submit’ button and it will open up an email with the form attached.
Please can you ensure all referrals are now sent to the teams electronically via the PDF referral form, rather than via fax.
All our assessment teams have implemented a robust telephone triage system, using NICE recommended clinical rating scales to ensure routine referrals are directed to the most appropriate service by our assessment teams, and to enable the teams to redirect those referrals not requiring a secondary mental health assessment or intervention. The new triage system means patients who do need to be seen by secondary services can be offered a face-to-face assessment appointment more quickly and patients requiring lower level interventions and assessments can be signposted to the appropriate primary care and third sector services in a timelier manner too.
