The Knowsley Integrated Community Frailty Team provides a two hour rapid response, for Knowsley registered patients 65 or over, who are at risk of hospital admission. The service is available from 8am to 7pm Monday to Friday and 9am to 5pm at weekends.
The aim of the service is to reduce unnecessary hospital admissions for people age 65 and over by wrapping care around them in the community.
The Integrated Community Frailty Team is led by community matrons with advanced skills in the frailty pathway, and is supported by nursing staff, therapists, a consultant geriatrician, social care professionals, medicines management and specialt falls assessors.
All urgent two hour response referrals/requests should be made through the enhanced clinical triage service, which is provided by:
Unplanned Care Direct (UCD) on Tel: 03003 230 240. UCD will act as a single point of contact for primary care colleagues who feel their patient needs an urgent two hour response in order to avoid an emergency admission.
