The Integrated Care Team provides a coordinated approach to health and social care for people living with long term conditions, a range of health issues, and frail or vulnerable older people.
The teams will focus on helping people to manage their conditions, improve access to information about healthier lifestyles and provide more care out of hospital so that people can stay as well and as independent as possible.
The team includes staff from social care, community and mental health, primary care, therapists, medicines management, health trainers and the voluntary sector.

This service is available across Liverpool and Sefton for people over the age of 18 years old and registered with a GP. Referrals to the Integrated Care Team are generally by a GP, social worker, district nursing team or community matron.
