What we do

The Community Specialist Palliative Care Team (Liverpool and South Sefton) provides a multi professional approach to caring for patients with advanced, progressive disease and links with other services and partner organisations.

Palliative care is an approach that improves the quality of life of patients and their families facing the problems associated with life threatening illness.

We strive to prevent and relieve suffering by means of early identification and the assessment and treatment of pain and other problems be they physical, psychological, or spiritual.

Patients may be offered an appointment for a holistic approach to an assessment by Clinical Nurse Specialists, who are dedicated in providing the highest standards of up to date specialist palliative care to registered patients of Liverpool or south Sefton General Practitioners (GPs)

We have an advisory role offering specialist expert advice on symptom control to patients, those closest to them and their GP, community nurses and other health and social care providers.

We offer practical advice on future care and attempt to anticipate needs of patients, aiming to prevent unnecessary hospital admission.

How we do it

Our team consists of Clinical Nurse specialists (CNS) in palliative and end of life care working alongside: 

  • District nursing teams in Liverpool and south Sefton
  • GPs in Liverpool and south sefton
  • Speech and language therapy services
  • Social care and social workers
  • Site specific Clinical Nurse Specialists (CNS) based in the hospitals
  • Allied Health Professionals
  • Marie Curie Hospice
  • Woodlands Hospice
  • St Joseph’s Hospice.

We work together ensuring palliative care systems, processes, education, training, and support are in place in whichever setting patients are being cared for.

We will respond to referrals in a timely manner, and all are triaged to the most appropriate person to help meet the needs of the patient.

Referrals are accepted from GPs, Community Nurses, Care Homes, hospital, and hospice discharges.

The patient must have complex palliative care needs to be referred to us and will only be declined if they refuse to accept the specialist palliative care offered.

CSPCT provides a seven day service, offering both face to face and telephone consultations.  Monday to Friday 9.00am to 5.00pm.

We work as part of a dedicated hub, the Integrated Merseyside palliative care team (Impact) service. Monday to Friday, 9.00am to 5.00pm which provides specialist palliative care advice to health professionals, patients, and carers via this telephone number: 0300 100 1002

Overnight, weekends and bank holidays any calls are answered by the IMPaCT Out of hours line. It is the same number 0300 100 1002

Outside of 9.00am to 5.00pm you will be given some additional options, please press the correct option to be put through to the right clinician (GP out of hours, Community Nurses, and Registered Nurses in Care Homes).
