Our service supports adults in Liverpool who have significant difficulties using mainstream adults' health services because of a learning disability. This could be for a number of reasons, such as problems communicating, problems with personal behaviours.
Our team is made up of specialist learning disability nurses, who are experts in working with people with learning disabilities, as well as occupational therapists, speech and language therapists, a specialist physiotherapist, psychologists and psychiatrists. We have an intensive support function within our team who can help in crisis situations as well as an out of hours on-call nurse service which is accessible at all times out of office hours including holidays and weekends.
We will assess you to help us understand what is causing your issues and how we can help. Following this assessment, we will put together a care plan that meets your individual needs. This could include physiotherapy, occupational therapy, speech and language therapy, psychology services and psychiatry.
We can work with people in their own homes, at college, at our team base at Norris Green Hub in Liverpool, or at day services across Liverpool.
This may be on a one-to-one basis, in a group or by supporting family members and carers. We also provide training for family members, carers and other professionals supporting adults with learning disabilities.
Our service is for adults age 18 and over who live in Liverpool or are registered with a Liverpool GP who have a learning disability.
You can contact our team directly for an appointment or you can visit your GP, who will be able to make a referral to our service for you.
Once we receive your referral, will then decide if we are the best people to offer support or if there's someone else who can help. If we can help, we'll contact you to arrange an appointment with our team.
