We provide assessment and rehabilitation for adults who have fallen or who are at high risk of falling. Our team consisting of physiotherapists, occupational therapists, and a part time podiatrist, supported by assistant practitioners.
We work with patients and their carers within their own home or in the local community. We offer an individual treatment plan to restore a person's independence and confidence and reduce hospital admission. This may include an exercise programme to improve balance and strength, advice on how to complete everyday tasks in a way to make them easier and safer, and guidance on foot care and suitable footwear to help prevent falls.
We are a Liverpool city wide service for people over 18 years of age registered with a Liverpool GP. Referrals are accepted from health or social care professionals, the patient must consent to the referral. We do not accept referrals which request equipment only.
Patients who have difficulty with their memory or retaining information may not be suited to our programme. Those referrals will be signposted to the other therapy services within our organisation who we work closely with.
