Who we are
The Specialist Palliative Care Team provides specialist palliative care to adults aged over 18, who are registered with a Knowsley or St Helens GP and who have a palliative diagnosis and complex specialist palliative care needs.
They also provide education and advice to other professionals on symptom control and end of life care.
They work closely with other health care professionals such as GPs and District Nurses.
This is a specialist advice service encompassing complex pain and symptomatic support, psychological and emotional support. The service is also able to sign post patients to other services.
The Specialist Palliative Care Team provides care to adults aged over 18, who are registered with a Knowsley and St Helens GP and who have been diagnosed with a palliative condition.
GPs, consultants and oncology teams, district nurses, hospital palliative care team, all hospices in the local area, other clinical nurse specialist who you might see in hospital, social services, nursing and residential homes.
Patients will initially be referred by a healthcare professional such as GP, district nurse or the Macmillan nurse from hospital. Once you have been assessed by the team and transferred back to your district nurse/GP, you can then re-contact the service yourself.
You will either be visited at home or invited to a clinic appointment where you will be assessed, and together with you, a plan of care will be completed. You may receive follow up reviews until your care has been completed or transferred back to the District Nursing Service. Again, this could be at home or in a clinic.
