Who we are
Our Healthy Knowsley Service can support you to make small changes to your lifestyle that result in a big difference to your health and wellbeing. The service will support you to set achievable lifestyle goals, allowing you to meet long term aims.
Our weight management services are for Knowsley residents or those registered with a Knowsley GP aged 16+.
We work in partnership with a number of services to ensure that you get the right support to meet your lifestyle goals. These include:
- Slimming World
- Volair Activity for Life Clinical Programme
- Volair Community Wellbeing Programme
- Smokefree Knowsley
- Knowsley CGL (drug and alcohol services)
You can self-refer to the Healthy Knowsley Service by phoning our free phone number 0800 0731 202 or you can talk to your GP or health care professional who can make a referral on your behalf.
Once we receive your referral, one of our wellbeing advisors from our Lifestyle Hub, will contact you by telephone to discuss the options available help you to achieve your goals. We will listen to how you want to make the changes and give you information on the services that can best support your needs. You will choose the support that you need and we will then make an onward referral to a specialist service or to see one of our wellbeing coaches.
Following your initial appointment with the Wellbeing Coach, you may have review appointments to track your progress and develop your initial plan to help you to meet your longer term goals.
You may also receive support from one of our Weight Management Dietitians and Cognitive Behavioural Therapist to help you to achieve weight loss goals.
We work in partnership with Slimming World and can support you to access the NHS weight management app if you prefer remote support.
All appointments are currently taking place as phone or video reviews. Your Weight Management Dietitian will call you at your appointment time. They will discuss your current lifestyle and develop an understanding of the support you need to enable you to meet your goals. They will discuss a range of weight management options and work with you to find the plan that best suits your needs. They will support you to develop a range of skills to help you to lose weight and maintain the weight loss.
