Who we are
Our dedicated community nursing service provides high quality nursing care and treatment to patients in their own homes (including residential homes) 24 hours a day, 365 days a year.
We treat patients who are housebound or require treatments where a home visit is necessary. The service offers professional advice, support, teaching and skilled nursing care to enable people to live as independently as possible with an acute or chronic illness/disability who have a nursing need.
We also run treatment rooms for patients who are able to leave their home and for whom it is clinically appropriate to do so.
The service supports patients aged 16 and over who are registered with a Knowsley GP. Examples of the care we provide includes:
- Holistic assessment to look at all areas of your physical and mental wellbeing
- Wound care
- End of life care
- Continence care
- Administration of injections / medications
- Ear care
We work closely with patients, carers, GPs and other health and social care professionals to help you to live as independently as possible.
You can be referred to the service by your GP or other health and social care professional. We also accept self referral.
Self referrals are also accepted from patients who have previously been known to the service in the last six months and require support for the same reason.
Following a referral, your community nurse will undertake an assessment to discuss your needs and agree a care plan with you.
