Who we are
Our Home Treatment Team provides intensive home support for adults with mental health problems in Halton to support them to manage their conditions in a community setting and prevent readmission to hospital.
You may be referred to us following an assessment by our Assessment Team, or following a stay on one of our mental health inpatient wards.
The team involves families and carers, using a flexible approach which provides an alternative to inpatient admission and supports early discharge from hospital. Our Home Treatment Team can visit you at home or in another community setting if you'd prefer.
The service is available from 24 hours a day, seven days a week, 365 days a year.
Our service is for adults with severe and complex mental health problems who need intensive support to manage their conditions in the community to prevent the need for admission to hospital.
As our Home Treatment Team provides a specialist service, you can't self-refer to the team and referrals will need to be made by a professional, such as a member of staff from Halton Assessment Team or your local mental health inpatient service.
If you are concerned about your mental health, make an appointment to see your GP. He or she will be happy to work with you to help you to manage mild to moderate symptoms of mental ill-health outside of secondary services.
It might be that you do need more specialised support. If so, your GP can make a referral to Halton Home Treatment Team on your behalf, who will assess your mental health needs and will be able to refer you onto us if they think we can help.
