Overview of Service
The Rehabilitation at Home Team aims to maximise the potential and capability of adults to achieve independence through intensive, short term therapy interventions, delivered in the home, following unplanned orthopaedic surgery at Aintree University Hospital Trust.
This team supports adult patients following fracture neck of femur surgery from the Orthopaedic wards at Aintree Hospital. They aim to maximise the potential and capability of patients to achieve independence through intensive, short term therapy interventions, delivered in the home.
The Rehabilitation at Home team select patients from Aintree wards according to agreed set criteria, which are flexible in order to meet the needs of Aintree. The patients move onto this pathway according to team capacity and thus the service does not operate a waiting list.
Patients return home as early as two to three days post op. Service provides OT/PT/Assistant input in the home and loans equipment to patients for short term use.
The service covers the Liverpool city boundary, South Sefton and Knowsley.
Service core hours are Monday to Friday, 8:00am to 4:30pm with individualised appointments outside these hours if necessary. The Rehab at Home Team undertakes needs-led weekend visits.
Patients having hip surgery in Aintree Hospital who are within the catchment area and fit the referral criteria are referred by the Multi-Disciplinary Team (MDT) of the hospital.
