Aims of the service

The community Later Life and Memory Service specialises in meeting the needs of older adults with mental illness and dementia. This dedicated team provides innovative, whole person intervention and treatment through a multidisciplinary approach. Collaborating with other agencies and professionals within the community, we provide rapid and responsive input that includes support, intervention and education.

Who do we see?

We accept referrals for people whose GP is in the area of Warrington. Warrington Wellbeing Service accept referrals for people who pay their council tax to Warrington.

Who is in the team?

The team provides a multidisciplinary approach with access to a wide range of professionals:

  • Consultant psychiatrist
  • Senior nurse practitioners
  • Nurse practitioners
  • Occupational therapists
  • Team manager
  • Support workers
  • Assistant practitioner
  • Administration staff
  • Social workers

Service hours

We can be contacted between 9.00am and 5.00pm, Monday to Friday (excluding bank holidays).
