The Halton Later Life and Memory Service is situated at the Brooker Centre in Runcorn. The service is aimed at people who are experiencing age related mental health difficulties, for example: depression and dementia.
The service consists of:
Assessment team of community psychiatric nurses who will complete a frontline assessment to establish the severity of your mental health difficulties.
Community mental health team who work with people who have enhanced and complex mental health needs. This team aims to promote independence in a number of ways to reduce the frequency and severity of any future crisis. The team comprises community mental health nurses (CPNs), occupational therapists, social workers and support workers.
Memory team works with people who have a diagnosis of dementia and have started memory medication. They provide regular reviews and support to service users and their carers. They run various groups for both service users and carers. The team comprises: community psychiatric nurses, occupational therapists and support workers.
Older person’s psychiatric liaison service based at Lancashire House, Warrington General Hospital. This service provides psychiatric liaison to service users who are inpatients at Warrington General Hospital or Halton General Hospital and who have a diagnosis of dementia or age related cognitive impairment. The service is run by a senior nurse practitioner.

Collaborative working
The LLAMS service work closely with service users, carers and all other agencies including the local Alzheimer’s Society to try to assist the service user and their carers to receive the necessary care, support and treatment required to maintain quality of life.
Referral process
Referral is via your GP, accident and emergency department or older person’s liaison service.
Upon receipt of a referral, the team will screen the information and where possible, contact the person who has been referred, or their carer, to take further details.
If the referral is appropriate for the team, the service user will be given an appointment for a detailed assessment and appropriate support within 10 days of receipt for routine referrals, or one day if the referral is urgent.
