Aims of the service
The Care Home Liaison Team specialises in meeting the needs of older adults with mental illness and dementia. This dedicated team provides innovative, whole person intervention and treatment through a multi-disciplinary approach. Collaborating with other agencies and professionals within the community, we provide rapid, proactive and responsive input for care homes that includes support, intervention and education.
Who do we see?
The team accepts referrals for people who live in residential care and nursing homes in Halton. We also carry out proactive visits.
What we do
Respond rapidly to urgent and routine referrals to carry out mental health assessments to promote early intervention and treatment.
Advise on the appropriate intervention and management that wherever possible enables the care or nursing home staff to continue to support and care for the individual.
Offer support and advice on issues relating to the management of symptoms and behaviours associated with mental illness,including dementia.
Liaise and collaborate with other agencies and professionals including: general practitioners, district nurses, community matrons and social workers to promote proactive patient centred care.
Review medication.

Within the Care Home Liaison service thereare two senior nurse practitioners with access to the following professionals:
- Consultant old age psychiatrist
- Occupational therapists
- Specialist mental health pharmacist
- Team manager

Service hours
We can be contacted between 9.00am and5.00pm Monday to Friday. We operate flexibly between the hours of 8.00am and 8.00pm. The service is not operational on bank holidays or weekends. We are here to support you in a range of ways including:
