The Assessment Team and Memory Clinic
We are a service who sees people because they have been referred by their GP with concerns about their memory.The team includes consultant psychiatrists, mental health nurses, occupational therapists and healthcare support workers.
You may be prescribed medication for your memory difficulties and if so, would be reviewed every six months.
The service aims to:
- Identify any memory difficulties through a comprehensive assessment.
- Liaise with other services to make sure you receive the best care and support available
- Carry out an occupational therapy assessment, if appropriate.
- Provide support and advice to both the service user and their carers.
- To assist service users to continue a fulfilled life.
Why are you experiencing memory problems?
There are a few reasons as to why you are experiencing memory problems.
These could include:
- A physical infection
- Anxiety, depression or high levels of stress
- A history of stroke or cardiac conditions
- A type of dementia such as Alzheimer’s disease
- Effects of combined prescribed medications.
You may not have noticed yourself that you are experiencing memory problems but other people such as your GP, family or friends may have done.
What happens when you visit us?
We advise you to bring someone who knows you well to your initial cognitive assessment. This could be a family member or friend.
The appointment will last approximately one hour, and we ask that you visit St. Helens hospital following the appointment to have an ECG carried out and your bloods taken.
If you use reading glasses or a hearing aid, you will need to bring them to the appointment. We also ask that you bring a list of any medication that you are taking.
A practitioner from our team will talk to you about your memory problems and carry out a memory test. They will also talk to you about strategies to help you manage your memory problems. If a carer attends the appointment with you, they will also be offered an assessment.
Following the initial assessment, you will receive an appointment to meet with a consultant psychiatrist to discuss a diagnosis.
They will also discuss options for treatment which could include medication. If you receive a diagnosis, you will be invited to a post diagnostic counselling session.
You may also be referred to group therapy. The groups include:
- Cognitive stimulation therapy
- Music and reminiscence therapy.
