What is the Enhanced Care Home Team?

Enhanced_Care_Home_Learning_Disability_Team_-_image_3.jpgThis is a new role developed in line with NHS Long term plan and following recent mortality reviews.

We are Learning Disability Practitioners who are based in the Liverpool Community Learning Disabilities team implementing the Enhanced Health in Care homes Framework for the learning disability population of Liverpool.

Our role includes:

● Identify people with a Learning Disability in care homes

● Improve health outcomes

● Reduce health inequalities for people living in care homes with a diagnosed Learning Disability

● Offer advise to support staff and other professionals.

Who is in the team?

The team is made up of two Specialist Learning Disability Nurses.

Multi-disciplinary working

We are part of the CLDT, we work alongside all professionals within Mersey Care Learning Disability services.

We will regularly feedback and review input with all CLDT members involved in each case (Nursing, Psychology, Psychiatry, SALT OT, etc.).

We can also provide advice and consultation to professionals in the CLDT and other agencies.

Liaise and provide support and advice to primary care networks and care home providers.

● To improve staff knowledge around Learning Disability Awareness

● To provide a consultation role to providers and professionals supporting individuals with aLearning Disability

● Enhance quality of life, inclusion, health,wellbeing and independence for individuals andthe people who support them

● Reduce hospital admissions and inappropriate referrals

● To prevent diagnostic overshadowing.

What can you expect from the Enhanced Care Home Team?

The Enhanced Care Home Team provides a consultation role to colleagues working in primary care, under the Enhanced Health in Care Homes Framework.

Specialist input may be provided in the form of:

● Seven day follow up assessment following hospital discharge in conjunction with community matrons

● Ensuring annual Learning Disabilities health checks are in place including health action plans

● Supporting with health needs assessments

● Making referrals to other professionals/agencies

● Providing training to staff teams and families

● Liaising with Learning disability nurses in hospital; sharing information regarding admission and discharge

● Signposting to appropriate services.
