The team is made up of nurses, mental health practitioners, and administrative staff all here to make sure we capture all your health needs in one place called the health assessment. The information in this leaflet aims to give you information about your review health assessment.
How often is your Health Assessment Review?
We are here for you, our team will offer a tailor made support package that suit you and your needs but we do need to complete your Health Assessments Review every:
- Six months for children under five years of age
- Once a year from when you are five years old up until your turn 18.
Once you turn 18 we will give you your health passport. A health passport is and overview of your general health which includes all your important health history.
Your review health assessment will take place with a nurse from the Knowsley Children In Care Team, but your health assessment won’t be the only time you see and hear from us. We will be in regular contact with you to review your health action plan and offer any additional support you feel you may need.
Your carer, parent, key worker, social worker, or GP may also ask us to visit or speak to you if they feel that you would benefit from further support.
It gives you chance to talk to us about any health worries you might have or to ask any questions you might have. No question is silly or unimportant so if you have a question, please don’t be too embarrassed or afraid to ask.
Things we will talk about include:
- Eating and diet
- Sleeping
- Development
- Activities, exercise, and hobbies
- Emotional wellbeing (including your feelings, worries, hopes and behaviours)
- Illnesses, medication and allergies
- Maintaining a healthy lifestyle
- Staying safe
- Safe and healthy relationships
- Transition to adulthood
- Any health goals you might have.
At the assessment, you can also have your height and weight measured.
If you’re with your carer or parent, we may also ask them some questions
Your health plan
As part of our discussion, we will work with you to agree what gets included in your health plan. This may include information about other people and services that can help and support you with your health needs to keep you happy and healthy.
Your Health Assessment can be offered at a place that suits you. This can be either at your school, children’s centre, health centre or at home and offered at a time that is best for you.
Confidentiality
If you want to talk to the nurse without any adults present, then let us know. It is important for you to have the opportunity to express your wishes, feelings, and views and if you wish to do this without another adult present that is okay.
What we discuss will be treated sensitively and in confidence but if something you share with us puts your safety or safety of another person at risk, we will need to share this with other professionals. If this happens, we will talk to you about it and let you know that this will happen and explain our reasons why.
