Community Matrons support patients who have complex long term conditions and currently have a very high intensity use of health care. With the Community Matrons advanced Specialist Nursing Care, these patients are able to remain at home longer and to have more choice about their health care.
The Community Matrons work pro-actively with the patients and carers to co-ordinate the individual health and social care a patient needs. Ensuring people get the right help at the right time.
Community Matrons are based in 4 localities across South Sefton; Seaforth & Litherland , Bootle, Crosby and Maghull. Each GP Practice has a named Community Matron and we provide care to patients within their own homes
Access to the Service is through a GP, although referrals are considered from all Health and Social Care professionals.
