The Judging Panel

A panel of judges will decide on the process of assessing achievement, shortlisting and nominating award winners. The panel will be chaired by a non-executive director of the trust and will include representatives from members of staff, service users and carers and external agency/organisations.

The closing date for entries is 4 September 2017.

The judge's decision is final.

Please gain the permission of your nominee.

We want to celebrate everything positive about the care we provide. By taking part, nominees agree to cooperate with the judging process and resultant publicity. This could include trust publications, press and media. Photographs and filming will take place at the awards ceremony. Please submit separate forms for each nomination. If you nominate the same person or team in more than one category, please ensure the supporting information reflects the criteria for that category.

A special Winner of Winners award will be chosen on the night by the awards audience. This award carries a prize value of £1,000, which must be used to enhance or improve the lives of our service users by contributing to the continuing role or project of the winner, as outlined in their application.

Tickets to this year’s awards will be priced at £20 per person to help cover the costs for the event.
