New courses for South Sefton residents mark Diabetes Awareness Week 2018

Diabetes Education in South Sefton

Do you have Type 2 Diabetes?  Take control of your diabetes.

The South Sefton Community Diabetes Team is based at Litherland Town Hall Health Centre. We deliver clinical care to people with Type 1 and Type 2 Diabetes, either in a clinic setting or at home if they are housebound. We also support people in managing their diabetes through education.

Diabetes and You is an informal first step diabetes education programme that is offered to people who are newly diagnosed with Type 2 Diabetes. Understanding your condition and how to manage it is an important part of diabetes care. Everyone who is diagnosed with Type 2 Diabetes is now automatically referred to the Community Diabetes Team and invited to attend.

The course is made up of two sessions delivered a week apart. Each session lasts for about two hours and covers the following:

What is Diabetes?

Healthy food choices

Which foods will affect blood glucose levels

How being active and increasing activity helps to improve diaetes and also general health

Finding support to stay motivated when making lifestyle changes

Diabetes Medication

Good fats and bad fats and how they affect the heart

Annual review

Diabetes and More is an informal next step diabetes education programme for people who have had their diabetes for more than six months. The main aim of this programme is to help people firstly identify any changes they need to make to manage their diabetes and to support them in setting goals to achieve this.

The course is made up of two sessions delivered a week apart. Each session lasts for about two hours and covers the following:

Hot Topics

What questions / concerns do you have about your diabetes?

What is important to you about managing your diabetes?

Types of carbohydrate and how this affects blood glucose levels

Portion sizes

What care should you expect over 12 months

What do your blood results mean?

Preventing / reducing risk of long term problems

Footcare

Managing diabetes during illness / holiday / special events (birthday, wedding etc)
