The Specialist Palliative Care Team support people and their families and carers when a patient has a palliative diagnosis. The team uses a holistic approach encompassing physical, social, psychological and spiritual wellbeing.
The team uses their knowledge and expertise to provide education, information support and symptom control for those living with disease, to help improve their quality of life, and that of their families and carers.
The team is attached to a group of of GPs and work closely with district nurses and intermediate care and other professionals and hospitals and hospices.

The service is available 24 hours a day, 7 days per week. During weekend and bank holidays your district nurses can access the service
You can access this service through referral from district nurses, GPs and other health professionals. The service responds to all new referrals within 48 hours. Patients and carers can call directly for advice.
