What we do

Community Intermediate Care Team

The Community Intermediate Care Team provides a multidisciplinary service for the population of South Sefton within the community.

The team aims to:

  • Prevent unnecessary hospital admission
  • Facilitate early discharge from hospital
  • Preserve and restore a patient's independence within their own home.

Urgent Care Service

The Urgent Care Service is integrated within the existing Community Intermediate Care Team (CICT). The service is available from 8am - 9pm, seven days a week.

The service will provide a two hour response to patients who have been referred by their GP, Community Matron, District Nurse or Community Therapist for urgent treatment.

The multidisciplinary team includes nurses, occupational therapists, physiotherapists, and health practitioner assistants.

The service supports a reduction in hospital admissions by managing deterioration of health or crisis for a maximum of 72 hours within the patients own home.
