Adult Community Occupational Therapy

What we do

Our Community Occupational Therapy Adult Rehabilitation Team works with patients, and where appropriate, their main carer to support a person to remain in their own home.

Our aim is to maintain or increase someone’s level of independence and ability to carry out daily living tasks. This is usually following illness or injury but also because of the effects of ageing or long-term/life threatening, physical, or medical condition.

We also support people to manage long-term and life threatening health conditions at home to avoid hospital admission. Occupational Therapy interventions can be delivered in the person’s home and local community and are based upon patient identified goals. 

We use the Care Aims Model for assessment and rehabilitation and work closely with other members of the community multidisciplinary team .e.g. physiotherapists, speech and language therapists, dietitians, podiatrists, district nurses and community matrons to ensure a holistic and timely approach to patient care.

Referrals are open to those over 18 years of age who are residents with a Sefton GP. Services are Monday to Friday; core times are 8am - 4.30pm with individualised appointments outside these hours, if necessary.

We accept referrals from any health professional who may be involved in the patient’s care. We do not accept referrals which request assessment for equipment only.
